I think all Rash victims start to transform right away, but that it takes a while before the changes begin to show + only a few survive this long enough to trollify. Remember how Mikkel could tell those skellies near their campsite had had the Rash but hadn't died of it? They weren't showing enough deformations, but some weird bumps that he said were a sign they were at "relatively early stage of the illness".
